An integral part of the Affordable Care Act includes the creation of health care exchanges to establish a marketplace for the purchase of affordable health insurance plans. States were meant to tailor their specific needs to a state-based exchange, but those who opt out will be obligated to have the federal government run an exchange for them:

WASHINGTON, D.C. -- Congressman Steve Cohen (TN-09) today announced that the National Eye Institute (NEI), an agency of the U.S. Department of Health and Human Services, has awarded a grant worth $375, 000 to the University of Tennessee Health Science Center (UTHSC) for vision research. Specifically, the research will focus on development of new blood vessels for the retina.
